Will An Aspirin A Day Keep Cancer Away? Maybe–But Not Just Yet

A study released early this evening in the medical journal The Lancet is going to create another discussion about the possible role of chronic aspirin use in the prevention of cancer.   And I suspect it is also going to raise a flurry of questions from some quarters about the type of evidence we need to have in order to feel comfortable with making potentially life-saving recommendations to millions of people around the world.

For me, it makes me wonder how we are ever going to answer the questions that we have to answer about the prevention of cancer, especially when it comes to readily available, reasonably inexpensive over-the-counter medicines and vitamins such as aspirin and vitamin D.

We should never forget that the story is as much about the quality of evidence and the risk of harm as it is about the inevitable headlines that this or that something or other is going to prevent cancer.

In this study, the researchers examined data from 8 previous studies that looked at aspirin as a means of reducing the chances of a vascular event, such as heart attack or stroke either as an initial preventive measure or a subsequent prevention strategy after someone had suffered an event.  They looked at studies that included patients who had been treated with daily aspirin for at least 4 years or longer. 

They then examined the death certificates of those people in the trials who were thought to have died of cancer, and examined more detailed medical records of patients treated in three trials in the United Kingdom so they could have a longer term evaluation of aspirin’s impact on deaths from cancer.

The researchers found 674 cancer deaths among 25,570 patients.  They found that overall, the risk of death from cancer was reduced 21% in patients who took daily aspirin compared to those who did not.

In the studies where they had individual patient information, there were 657 cancer deaths among 23,535 patients.  Here, aspirin reduced deaths from cancer by about 18%, due mainly to a decrease in deaths after 5 years of aspirin treatment.  Decreases in deaths were seen mostly in gastrointestinal (GI) cancers.

When the scientists examined records for those with a 20 year follow-up, they found that the “aspirin group” had 20 % fewer cancer deaths overall, 35% fewer deaths from GI cancers but no decrease in what we call “hematologic cancers,” which are known to you as lymphomas and leukemias among others.  The greatest benefits were seen in those people who were treated with aspirin for 7.5 years or longer.  In this 20 year group, there was a “delayed” or latent period until the deaths declined, with decreases in deaths from cancers of the esophagus and pancreas seen at about 5 years onwards, while decreases in deaths from stomach and colorectal cancer did not appear until about 10 years.  The researchers also reported that in absolute terms, the risk of dying from cancer was decreased by a significant 7.08% at age 65 and older.

Most of the decrease in deaths in patients who had cancers other than GI cancers and were followed for at least 20 years occurred in patients with lung cancer, and most of those had the type of lung cancer called adenocarcinoma, which is the most common form of lung cancer in the United States.  There was some decrease in prostate cancer deaths late in the follow-up, but this was not statistically significant and was seen in patients who had taken aspirin for 7.5 years or more.

One observation that I think deserves note is the fact that most of the decreases in deaths were seen in cancers that are what we call adenocarcinomas, which are cancers that form in “glandular” organs.  Another point that is important is that most of the participants in these trials were male, and therefore no comment can be made one way or the other about common cancers in women, such as breast cancer or gynecologic cancer.  Another very important point is that the dose of aspirin was 75 mg a day or greater, and increasing the dose did not increase the benefit.

Does this study mean that everyone age 40 or 50 and over should start taking aspirin?

Although the study authors make the case that there are several reasons to believe that this trial can be generalized to larger populations, I don’t think that this can be turned into a recommendation for everyone to start taking aspirin.

Yes, there is research that shows aspirin can reduce the risk of colorectal cancer.  Yes, this is an excellent study that breaks new ground and provides us with important and valuable information.  Yes, it may turn out sometime in the future that aspirin will become a valuable addition to our ability to reduce the risk of death from cancer-for which we have few effective proven alternatives at present.

But aspirin has side effects, my friends.  It causes bleeding in the GI tract and can cause bleeding in the brain.  Before we doctors started making recommendations about the use of aspirin in people at risk of heart attacks or those who had one, there were prospective trials done so we would understand not only the benefits but also the risks.  That information is not available from this particular report (that is not a criticism of the authors’ work, just an observation).

We have faced these issues again and again.  We plow the fields to find information where we can when we don’t have the right types of clinical trials to answer the questions.  But the problem is that we frequently try to point the way through more or less indirect means when we don’t have direct information.  And then there are the situations that have occurred not infrequently where such “retrospective” analyses produce results that don’t stand up when carefully designed prospective trials are conducted.

What would be more definitive and exact would be trials that are done specifically to address the question at hand, randomized going forward, and controlled for all sorts of factors.  That is what we call a randomized, prospective trial.

What we have here-and let me be very, very clear: I am not criticizing the quality of research done by the authors-is an analysis of other studies done originally for measuring other outcomes.  What we need is a prospective trial asking the question specifically about the impact of aspirin on cancer deaths.  But the real question is whether any such trial will or ever could be done, especially when you consider that it would take over 20 years to complete.

So we are left in a bit of a quandary, just as we are with vitamin D and have been in the past with other vitamins (until the randomized trials were actually performed).  That means experts are going to have to digest this information, and decide whether or not it justifies subjecting millions of people to a recommendation that they take a baby aspirin every day.

I don’t know when that is going to happen, yet I am certain there are some/many of you out there are going to charge ahead and take aspirin every day once you hear about this study.  And there are some of you out there who do that and will end up in a hospital emergency room or worse as a result of that decision.   But we will never know who you are.

So that is the dilemma we face as we write and talk and broadcast the results of trials such as these.  I can be as cautious as I want to be, and tell you I would not recommend you start taking a daily aspirin at this time, yet there will be the inevitable headlines and news stories that will shortchange the cautious part and propel you forward to an action that could be harmful, based on evidence that is good in a scientific sense but not sufficient in a “patient recommendation” sense.

As I have said before, and will say again: proceed with caution with “eyes wide open.”  I have no doubt there will be more information forthcoming on this topic that may make the decision just a bit easier and perhaps a bit better informed.

Stop Smoking Study Could Go Further

(Media-Newswire.com) – A recent breakthrough shows that a brain scan can predict if a smoker can kick the habit. A local scientist believes that’s just the start.

Glen Hanson, the director for the Addiction Center at the University of Utah said the study focuses on activity in the frontal cortex. It’s a part of the brain where you “process information… and it’s a big inhibitory control of the brain.”

The study showed the smoker’s frontal cortex would “light up” when asked about how they felt about quitting. That information indicates the cortex is working and the chances are good the smoker would kick the nicotine habit.

Hanson believes this study is just the beginning. “We might try to develop strategies or mental rehab that will strengthen that brain system,” he said.

He says they could also develop medication that could help people stop smoking.

However, Hanson says more studies need to be done and more questions must be asked before any solid conclusions can be made.

“Like ‘How does smoking make you feel?’ and ‘Would you like to eliminate that feeling?’” he said.

Hanson says those answers could lead to better predictive behavior. He also believes scientists need to look at other parts of the brain during the scan.

Hanson says that could also make the prognostication more accurate.

Now granted, this is just an estimation but it has triggered legislation in virtually every state to reduce the amount of second hand smoke that we are now exposed to.

Lets’ take a look at a few other startling statistics about passive smoking. Did you know that children are exposed to more second hand smoke than adults? And did you also know that the harmful effects of second hand smoke can last their entire lives?

Here are just a few health issues that are raised when children are exposed to passive smoking. They can suffer asthma, increased chances of bronchitis and pneumonia, and are more prone to ear infections and other respiratory diseases. Start thinking about what passive smoking is doing to people, especially children. Quit Smoking and Sleep Like a Baby

Have you ever observed a sleeping baby? Unless the baby is suffering from colic, a baby will sleep peacefully throughout the night. You can carry the baby, move it, change its clothes and do almost anything that will not cause any violent movement that will awaken it. You can even hear him snore; honestly yes, when my daughter was still a baby, she snores lightly and seeing her sleeping peacefully hearing her gentle snore is enough to give you peace and inner happiness.

So why am I discussing how a baby sleeps? Well when I was a smoker, I never slept like a baby and if you are a smoker reading this post, I’m pretty sure you are also not sleeping like one. For a smoker, the night is not a moment to find peace, relaxation and rest. There were times when I feel the tightness in my lungs and I could not breathe. There were times when my body was trying to cleanse my lungs and I find myself coughing severely. And there were times when I was haunted and my mind keeps on worrying about the smoking related disease I could acquire. Every pain I feel in my chest and in my head produce undying imaginations of me having a dreadful lung cancer or a tumor developing in my head. These are just a few things that never made me sleep like a baby. Although some smokers are happy during the night time because they could save themselves from smoking.

If you are also suffering or undergoing the same situation that I did as I mentioned above when I was still a smoker, isn’t it time for you quit smoking? Tobacco contains nicotine which is a very addictive substance.  <—- Good answer to the title of this post don’t you think?

Usually a lot of people begin smoking through peer pressure, learning from family members who smoke, especially parents or just want to try it out. A lot of guys especially teenagers see it as fashionable and “cool” so to speak to smoke.

Peer pressure is hard to avoid but sometimes its best to just walk away. Parents are supposed to be the main role models for their kids, but when kids grow up in an environment where the parents smoke then its more likely for the kids to start smoking at an early age. In very rare situations do children growing up in such an environment end up being averse to smoking. Stealing a cigarette or two from mum or dads pack, giving it a try, we’ve all had our fair share of being rebellious and of curiosity.

The question that comes to mind is who is paying the price now?

Correct me if I am wrong, most people don’t like smoking the first time they try it, I sure hated it I claimed I would not do it again but fell prey to peer pressure so much for never smoking again, how was your experience?  If you’re like 99% of other smokers you probably were feeling excited, your heart racing you were doing something that you know you weren’t supposed to do, but human nature has that element of curiosity which makes us want to try new things, ever heard the expression curiosity kills the cat? Well in this case curiosity seriously kills the cat!

That first puff, do you remember what it was like? With smoking though, despite the fact you don’t like it, that first puff you took has you either ‘Hooked’ for a long time or just gave you the idea that you will never smoke in your life again. Some successfully never smoke again and other are simply weakened by it and the more the smoke the more irresistible it gets.

Let me not waste your time, I had been smoking for almost 13 years, I know how hard it is to break free from the cigarettes, I wont lie to you they are really really addictive. Not only that but they do more damage than good. So why cant you quit.

Always cigarettes on my mind. If there was a few remaining in my box, I had to venture out and get a box, either on the way to college, or if I had a day off I would make sure it was high on my “Mental To-Do-List”:

Get smokes…..Secured my fix…..now come other tasks.

During the day on the way to work have a smoke. At work somehow take a few minutes off to have a “Smoke break” usualy with another colleague that smokes, and then get back to work. After lunch have a smoke, I remember those were a relief, just the thing to seal your apetite 100% after a good meal. Then on the way back home another cigarette maybe squeeze a few before that, at home another smoke “Ahhhhh your finally home what a way to relieve all that stress off a hard days work”.

Ashtrays all over your house, some full some half full, the stench of smoke on your clothes, go out for coffee with friends, if you asked me it was more like who can outsmoke the other whilst downing coffee. And then finally just before you sleep have a cigarette while the rest of the world is slowly lulling away into the darkness of the night.

Can you relate to any of this?

If yes then I hope you understand what I mean by “Smoking is a lifestyle“. Its a way of doing things and if you’re going to live that lifestyle then you’re going to need a box or two everyday just to keep this lifestyle going. Not to mention a new set of lungs to keep up with the lifestyle. (Amazingly enough with todays advanced medicine getting a new set of lungs is possible!!!!)

Now imagine your day without it? What would you do without the smoke breaks at work? How would you react to your colleague inviting you to a smoke break and you say no I quit smoking?

Let me get the main point to you here, you want to quit you got to change the above lifestyle, and if you do things differently then you got to start seeing yourself doing it with cigarettes out of the equation.

What is your number one excuse for not quitting?

Today I just have a simple question for you and a few suggestions from my personal experiences. I want to find out what the number one excuse is that keeps you from quitting smoking?

Ask yourself this question:

Why have I not quit smoking until now?

Im expecting an answer that goes something like this:

I have not quit smoking until now because, every time I quit, I go back home and see my spouse smoking, or at work my colleagues invite me for a smoke break even when they know I am trying to quit so I break down and can’t resist the temptation.

Or even

I still have not quit even though I know its bad, because I have never ever tried to quit.

Answers like the ones below are really not answers:

1. I am addicted or I am an addict is not an answer, all smokers are hooked on the stuff thats why they struggle to quit (duh)!!!

2. I start craving for them, trust me I craved and felt like crying at times watching the person next to me smoke and just knowing I cant smoke.

The whole purpose of this is to find the weak spot that gets you to go back, hopefully we can go from there to find something to aid you quit. I sincerely want to help you find that answer to help you quit smoking.

I intend to create an application of some sorts that can help you overcome this weakness, so I am looking for a little data to work on. Your help will be appreciated.

Advice from an ex smoker

Just a few words from an ex-smoker to end this post, I did it and so many others have, so there really is no reason why you cant.

I honestly believe hypnosis can help, clinical tests even prove this to be true see this link  (Hypnosis increases your chances of quitting by 66%).

This is why I  have included a quit smoking hypnosis audio program under the free quit smoking aids section. I actually used this myself and let me share with you why I know this works.

I listened to it a couple of times and then tried quitting. Every time I found myself in a situation where I was craving I would take a deep breath and exhale by blowing the steam out slowly. This is behavior I have never done before, I suspected the hypnosis may have worked and listened to it without falling asleep or relaxing as you are told.

I wanted to listen to what suggestions were being made, turns out that one of the suggestions was to take a deep breath and blow it out slowly every time you find yourself in a situation where you feel vulnerable. It didn’t take away the craving but it made it that much more bearable so I managed to stay quit long enough to have quit for good.

I was amazed it worked, of course you got to listen to it at least once a day for 30 days, especially if you are the skeptical type. I did 2 sessions daily for 2 weeks to see these results.

There really is no reason to burn your pockets on any quit smoking products to quit, a combination of hypnosis with a little will to end the habit of smoking from your side is all you really need.

Good luck and remember you only fail when you give up….don’t give up on quitting!
